How does identity theft happen?
Identity thieves steal or get Social Security cards and numbers, birth certificates, immigration documents, or personal information that is unique to you.

The Federal Trade Commission has a great identity theft resource to get you started. You can print out helpful checklists. You can also get a personal recovery plan created for you automatically.
- Call the companies where you know fraud occurred.
- Place a fraud alert and get your credit reports.
- Report identity theft to the FTC.
- You may choose to file a report with your local police department.
How do I notify state government agencies about identity theft?
- Iowa Department of Transportation (DOT). Use the DOT Identity Theft Complaint Form if you believe someone has a driver’s license or state ID card using your identity.
- Iowa Attorney General. Their Identity Theft page offers advice to get started. It also provides their contact information. You can also apply for the Identity Theft Passport .
- Iowa Department of Revenue. Use this Identity Theft form if you’re a victim. You can also report a person or business you suspect of fraud
- Iowa Workforce Development. Report identity theft of unemployment benefits with their web reporting form.
